Site induction form
Everyone who comes to work on site needs a site induction before they start. Fill in the site and the person, tick the topics as you cover them and print a record for both of you to sign.
What the law says
CDM 2015 makes the principal contractor ensure that a suitable site induction is provided (regulation 13(4)(a)). Every contractor must also make sure each worker under their control gets a suitable site induction where the principal contractor has not already provided one (regulation 15(9)(a)). On a job with only one contractor, that is you.
Who gives the site induction?
The principal contractor on a project with more than one contractor; otherwise the contractor. It is usually given by the site manager or supervisor, before the person starts work.
What should a site induction cover?
The site layout, welfare, emergency arrangements, first aid, site rules, the main hazards from the construction phase plan, permits in use, and the RAMS for the person’s own work. The checklist above follows that order.
Do I have to keep a record?
The regulations do not set a form, but a signed record is the easiest way to show the induction was given and understood. Keep it with your construction phase plan.
